Job description: Flemingdon Health Centre is a registered charity and an incorporated not-for-profit Community Health Centre (CHC). We provide a range of health-related services based on the social determinants of health and community engagement models. We are primarily funded through the Ministry of Health and Long-Term Care.

Purpose:

As part of the Community Engagement team, the Community Health Worker- pre-adult engagement (CHW) will be part of the Health Promotion (HP) team responsible for the efficient and effective delivery of HP programs and services in FHCs catchment areas. The Community Health Worker will work from both sites to deliver an array of programs and services.

Key components:

  • Bring a Community Engagement perspective to organizational planning within the HP team
  • Outreach to groups and organizations in the region to build communities of practice
  • Undertake an outreach strategy in the TCLHIN, to foster linkage e Plan, develop, implement and evaluate HP strategies, programs and services addressing the social determinants of health
  • Work collaboratively within a multidisciplinary environment in the delivery of activities and services across the organization
  • Build and maintain partnerships with residents and other organizations and providers
  • Maintain current programs and partnerships that have been developed by the current CHW

Employment terms: One Year, Pregnancy/Parental leave Contract (35 hours/week, with the expectation of working some evenings and weekends)

Salary Range: $49,969 – $59,427 in addition 4% vacation pay

Expected Start Date: February 2021

Reporting to: Manager, Population Health and Wellness and/or designate

Location: Flemingdon Health Centre Sites

Qualifications, Skills and Proficiencies:

  • Bachelors degree in a health or social science discipline from a recognized University and/or an approved equivalent of education and related experience
  • Three to five years’ work experience in a community based, inter-disciplinary environment
  • Passionate about community health and health promotion
  • Thorough knowledge of and demonstrated proficiency in the practice of health promotion and community engagement
  • Demonstrated experience in outreach
  • Experience planning, implementing and evaluating programs for emerging populations
  • Experience with intake, orientation, and registration of new clients
  • Ability to understand and respond to changing community needs
  • Ability to work independently and collaboratively in an inter-disciplinary team environment
  • Excellent written and spoken English communication skills
  • Experience working with a diverse community
  • Ability to work flexible hours including some evenings and/or Saturdays
  • Knowledge of catchment areas is considered an asset
  • Ability to speak languages other than English an asset

Responsibilities include:

General:

  • Produce health education materials as required, and prepare and distribute relevant resources
  • Liaise with relevant agencies serving the community to create and maintain contacts with individuals and networks
  • Write reports, maintain files, and, as required, develop documentation/databases necessary for evaluation
  • Maintain knowledge of changing demographics and evolving needs of those within the TCLHIN and ensure incorporation of health promotion advances into practice
  • Other duties as assigned by Manager, Population Health and Wellness or designate

Navigation and Case Management Support- Intake

  • Intake of new clients, including registration and liaising with primary care providers
  • Monitor internal referrals, and ensure client’s exposure to all facets of the centre
  • Refer callers and community members to appropriate community resources and services
  • Participate in enhancing the centre’s relationship with stakeholders and partnerships
  • Work closely with primary care providers to outreach to the community of clients
  • Provide navigation support to clients, including information and referral to local services
  • Provide brief crisis counselling to clients and community members

Programming:

  • Plan, implement, monitor and evaluate health promotion programs and strategies for emerging populations
  • Maintain up-to-date work plans and budgets
  • Use evaluation methods and reporting tools to gather and maintain statistical information on activities and programs
  • Work with students, volunteers and/or community members
  • Share programming evaluations and reflections with HP and Community Engagement teams

Community Engagement:

  • Support broad-based community engagement to advocate for social change in the broader context
  • Explore and expand community development approaches
  • Establish advisory groups to involve members of the community in planning
  • Act as a resource to the community and FHC staff by establishing links with external community groups, agencies and organizations
  • Identify community members willing to participate in and/or assist with FHC’s community engagement and/or development activities
  • Collaborate with stakeholders in the community to identify and respond to needs
  • Participate in internal and external committees

Note: This is a bargaining unit position with UFCW. Terms may be altered as a result of collective bargaining.
