Job description: Derbyshire County Council are currently looking for a Locum Qualified Social Worker to join their Hospital Team.
The ideal Social Worker should have prior knowledge and experience of a working within an Hospital Team. The key to being successful in this role is to join with great confidence and you will be hitting the ground running and working alongside a number of teams.

Your responsibilities working within the Hospital Team:
- Undertake person-centred assessments with service users
- Plan for the provision of support and services with service users to assist them in meeting their outcomes to live as independently as possible
- To gather sufficient information about service users to be able to accurately assess their needs against eligibility criteria
- To work as part of a multi-disciplinary team
Requirements of you working within the Hospital Team:
- A recognised Social Work qualification (Degree/DipSW/CQSW)
- 2 years post to qualifying Degree
- A valid Social Work England registration
- Have competent knowledge and experience working within an Hospital Team
Location: Derbyshire
Umbrella pay rate: £28.00 per hour
