Job Description:

The Social Worker will actively participate in the delivery of care and ongoing program development for the Paediatric Diabetes Education Centre. The Paediatric Diabetes Education Centre is comprised of a multi-disciplinary team that assists children/adolescents (aged 0 – 18) and their families as they adjust to and cope with a diabetes diagnosis. The Social Work position involves working with an inter-professional team, conducting psychosocial assessments, and providing mental health support to help children and their families lead active and healthy lives.

Responsibilities include:

  • Individual and family psychosocial assessments
  • Provide child and caregiver/family counseling
  • Case formulation and treatment planning with inter-professional team members
  • Collaboration/communication with the inter-professional health care team is essential
  • Consult and collaborate with community partners for treatment planning
  • Actively involved in developing and executing program initiatives
  • May include the supervision of trainees and learners
  • Integrates clinical and research responsibilities as the team is required to demonstrate an evidence-based approach to this clinical activity and to create, utilize and/or disseminate new knowledge
  • Collect and evaluate metrics

Qualifications

  • MSW or BSW, registered with the Ontario College of Social Workers and Social Service workers required
  • 2 – 3 years’ experience with paediatric patient care populations
  • Basic diabetes knowledge
  • An understanding of the psychosocial impact of living with a chronic illness
  • experience with individual and family psychosocial assessments
  • group facilitation experience preferred
  • excellent assessment, counseling, problem-solving, and community-resourcing skills
  • demonstrated ability to work well both autonomously and within a multi-disciplinary team
  • strong interpersonal, and communication skills
  • strong leadership skills
  • excellent project management and program development abilities
  • demonstrated ability to work effectively under pressure
  • demonstrated facilitation and consensus-building skills
  • must be able to meet the travel requirements of the position
